California law requires notification of any person whose confidential personal information may have been compromised as a result of a security breach. Many of us collect data that may contain confidential personal information. It is essential that we know about such data and where they are located in the event of a security breach. For this reason we need everyone in the department to review the attached memo and complete the survey. If you don't have confidential personal data, you only need to check the relevant box(es) in Section A. If you think this does is not a serious issue and can't happen here, I refer you to the news item on the UC Berkeley website athttp://www.berkeley.edu/news/media/releases/2004/10/20_breach.shtml.

It is both a law in California (SB 1386)and a University of California policy that individual users of computers are responsible for eliminating third-party personal information on their systems, and for ensuring the security of third-party personal information that must be retained for business purposes. The department urges everyone to examine their files periodically and erase any information that includes a person's name (first name or initial and last name) in combination with any of the following: Social Security number, California driver's license or identity card, financial account information and access codes, and credit card information and access codes.
